Responsibilities
- Perform initial evaluations and develop PT treatment plans for residents requiring rehabilitation services.
- Implement evidence-based PT interventions to improve mobility, strength, balance, and functional independence.
- Reassess progress, adjust treatment plans as needed, and document outcomes in medical records per policy.
- Collaborate with PT assistants, nursing staff, and other disciplines to coordinate multidisciplinary care.
- Educate residents and families on home exercise programs, safety strategies, and discharge planning.
- Ensure compliance with facility policies, patient safety standards, and regulatory requirements.
Requirements
- Doctor of Physical Therapy (DPT) or physical therapy degree from an accredited program.
- Active physical therapy license in Pennsylvania or eligibility to obtain licensure.
- Ability to work in a skilled nursing facility setting and manage a caseload in a travel assignment.
- Strong written and verbal communication skills; proficient in documentation and patient education.
- Eligible for temporary travel assignment and able to adapt to new teams and settings.
