Responsibilities
- Assess appropriateness of admission status and level of care using InterQual criteria.
- Provide discharge planning services, coordinating with patients, families, and community agencies.
- Collaborate with the interdisciplinary team in assuring appropriate utilization of resources and expediting high-quality patient care.
- Participate in utilization review and coordinate care across related services.
- Maintain documentation in the electronic medical record; use basic computer tools (Word, Excel, Outlook) as part of daily workflow.
Requirements
- Active Registered Nurse license (California) or eligible to practice in California.
- Knowledge of InterQual criteria; Medicare Conditions of Participation knowledge preferred.
- Experience in case management or utilization review is preferred.
- Proficiency with basic computer tools (Word, Excel, Outlook) and Internet navigation.
- Ability to perform duties in a home healthcare - hospice setting; willingness to work ten-hour shifts and participate in occasional weekend rotation.
