Responsibilities
- Administer prescribed medications and treatments per physician orders and facility policy
- Monitor residents’ vital signs and report any changes to the supervising nurse
- Assist with activities of daily living (bathing, dressing, mobility) and provide comfort measures
- Document nursing assessments, interventions, and outcomes accurately in the charting system
- Collaborate with RNs, physicians, and other team members to coordinate comprehensive resident care
- Ensure infection control, safety, and privacy standards are followed; respond to emergencies per policy
Requirements
- Active LPN license in New Hampshire, in good standing
- Graduation from an approved practical nursing program
- Ability to work evening shifts in a long-term care setting
- Strong communication and documentation skills
- CPR/BLS certification preferred
