Licensed Practical Nurse (LPN) – Long-Term Care, Waterloo, NY | Start 07/21/2026 | Duration 14 weeks | Weekly pay $1,296–$1,412 per week | Guaranteed hours: not guaranteed | Shift: 5x8 evenings | Setting: Skilled Nursing Facility | Travel - Temporary Assignment. This travel assignment is managed by Healthforce Healthcare LLC. The LPN will provide care in a long-term care setting under the supervision of a registered nurse and will rotate through day/evening/night schedules as needed.

Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of the RN.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and interdisciplinary team.
- Contribute to the development and revision of an age-appropriate, culturally relevant plan of care; involve the resident/family and healthcare team.
- Collaborate with physicians, staff, and ancillary teams to ensure comprehensive care and timely referrals as needed.
- Maintain current knowledge and competency in nursing practice; participate in orientation and staff mentoring as directed.
- Demonstrate knowledge of growth and development principles for adult and geriatric patients; adhere to safety, infection control, and documentation standards.
Requirements
- Licensure: New York LPN Licensure (active)
- Certification: BLS (American Heart Association or RQI)
- Experience: Minimum 1 year as an LPN; prior LTC facility experience preferred
- Education: Completion of an approved practical nursing program; current LPN credential
- Ability to work in a long-term care setting and participate in rotating shifts; orientation may start on a Tuesday or Thursday
