Responsibilities
- Provide direct patient care to home health patients, including assessment and treatment planning
- Conduct initial evaluations and ongoing progress assessments to guide care
- Design and implement goal-directed, individualized PT plans to improve function and mobility
- Deliver evidence-based interventions, document progress, and adjust plans as needed
- Collaborate with the care team and communicate goals and progress to patients and families
- Maintain timely, accurate documentation and ensure compliance with applicable standards
Requirements
- Active Physical Therapist license in Montana or eligibility for licensure
- Demonstrated ability to perform direct patient care in a home health or hospital-side setting
- Strong communication and documentation skills, with the ability to work with a multidisciplinary team
- Willingness to commit to a 16-week travel assignment and the 4x10-hour shift pattern
