- Assist with activities of daily living, hygiene, mobility, nutrition, and wound care as indicated.
- Document care and progress accurately in the resident charting system and update care plans as needed.
- Collaborate with the interdisciplinary team to ensure safe, comprehensive, high-quality care for residents.
Requirements
- Active Licensed Practical Nurse license in New York.
- Graduation from an approved practical nursing program.
- Current Basic Life Support (BLS) or CPR certification.
- Prior experience in long-term care or geriatric settings preferred.
- Ability to work in a fast-paced long-term care environment and follow established care plans and policies.
- Must be able to pass background check and drug screening.
Education
- Completion of an approved practical nursing program.
